I've deliberately left the locations out, but it shouldn't be hard to find the creatures, given the type and the level. I'm sure I've missed a few mobs out there, particularly those from instances. The level is important, only creatures of those levels have any chance to drop the given items.
Generally Swarm mobs have a lower chance to drop items, Signatures a slightly greater chance, and Elites a much greater chance.
